Starting behavioral health treatment may seem complex and intimidating at first, but it doesn’t have to be. The Meadows Psychiatric Center works hard to ensure the assessment and admissions process is as smooth and efficient as possible.
Admission to The Meadows
Assessment and Referral Center
Staffed with compassionate behavioral healthcare professionals, the Assessment and Referral Center is accessible 24 hours a day, 7 days a week. We provide assessment, evaluation, level-of-care determination and referrals to community resources based on clinical need. Assessments are confidential and provided at no charge. Admissions
We welcome referrals from psychiatrists, psychologists, social workers, case managers, employee assistance programs (EAPs), insurance companies, managed care organizations, MH/IDs, probation officers, friends, family members, self-referrals and clergy. Referrals are confidential and can be made by anyone.
The Meadows Psychiatric Center does not discriminate in accepting patients based on ethnic or national origin, race, religion, disability, age or gender.
We are an approved provider for most insurance companies and managed care systems. If you would like to make a referral or would like more information, please call 800-641-7529.

To request your medical records, please complete the AUTHORIZATION FOR RELEASE OF PROTECTED HEALTH INFORMATION and send it to The Meadows Psychiatric Center by either email or fax.
